職責 *

- Provide rehabilitation services, self-management programs, psycho-social support services, volunteer services, health education activities, and organize patients support groups in order to attain personalized care and an integrative rehabilitation
- Build partnerships with other disciplines, clients, caregivers, volunteers, and peer support programs to provide quality person-centered holistic care services
- Supervise and conduct on-the-job training for program workers and other supporting staff to ensure service quality

要求技能

- Recognized Bachelor degree in Psychology or Counselling Preferably with experience in services for people with chronic health conditions
- At least 2 years of experience in working with persons with disability, chronic illness, their caregivers and related community organizations is preferred
- Self-initiated, independent, flexible and good interpersonal skills
- Good command of both Chinese and English
